Is over-the-counter drug availability in the U.S. at all unsettling?

So I was chatting with a Eurofriend and I mentioned NyQuil. He said he never heard of it and went on to explain that over-the-counter drugs weren't really a thing, or were extremely niche in his country. When I showed him a pic of a Walgreens drug aisle, the reaction was pretty much 'wtf.'

Is this a regional thing, or is the drug availability for minor health issues in America at all concerning?
